Why reinvent the wheel when there are already some very handy checklists out there that already exist? Below are premade checklists that you can view and consult to create your own.

Here are some blank templates you can use to create checklists, notetaker forms, reaction sheets, and the test taker copy of the task list.

Your users will get stuck. They'll ask for guidance and you'll want to help them, but that is not what you want to do when conducting a usability test. To practice talking with test takers, here are some helpful guinelines for facilitating conversation, asking reflective questions, and guiding them without giving away anything.
